Purpose

Use the Finding Services tool to keep track of contacts for local agency services and the responses you receive.

Tips

»	Use this tool when you need some help or service from an agency.
»	Begin by writing a description of the service you need, then list agency names and contact information with the services that match those you need.
»	Determine if you are eligible for services by calling or visiting the agency and then make notes about their services and if you want to set up the service.
»	Review your information when getting ready for you IEP meeting so you can plan for agencies that may be helpful to you in the future.
