Purpose
Use the Commitment Card to set goals for yourself and set times to monitor whether you meet your goals or not. You can use the card to monitor your results and keep track of your success.
Tips
» You can list up to 5 behaviors to work on using this tool. These behaviors are commitments you are making to yourself to do.
» Decide when you want to check yourself. You can list up to 8 classes, time periods, or days on this tool.
» Print out the card and keep it with you to track your success Check off the number for each commitment you meet.
» To help evaluate your success, you can add up the number of commitments you meet and those you don't. You can make changes the next time you use this tool.
» You can have your teacher help you by signing the card, or you can use it privately.
