Purpose

Use the Transition Planner to help you determine your goals for the future, think about resources you will need, and plan your school transition meeting.

Tips

»	Use this tool when you are planning for your IEP transition meeting.
»	Enter the outcomes you hope to have and how you think the school can help you get them.
»	Enter community resources that you know will be able to help you now and once you have graduated.
»	List the people you want at your IEP meeting.
»	List the things you want to talk about during your IEP meeting.
